Leeches are annelids comprising the subclass Hirudinea. There are freshwater, terrestrial, and marine leeches. Like the Oligochaeta, they share the presence of a clitellum. Like earthworms, leeches are hermaphrodites. Some, but not all, leeches are hematophagous.
The European medical leech, Hirudo medicinalis, and some congeners, as well as some other species, have been used for clinical bloodletting for thousands of years, although most leeches do not feed on human blood, but instead prey on small invertebrates, which they eat whole.
Haemophagic leeches attach to their hosts and remain there until they become full, at which point they fall off to digest. A leech's body is composed of 34 segments. They all have an anterior (oral) sucker formed from the first six segments of their body, which is used to connect to a host for feeding, and also release an anesthetic to prevent the host from feeling the leech. They use a combination of mucus and suction (caused by concentric muscles in those six segments) to stay attached and secrete an anti-clotting enzyme, hirudin, into the host's blood stream.
Some species of leech will nurture their young, while providing food, transport, and protection, which is unusual behavior amongst annelids.

LEECH THERAPY: AN OVERVIEW


The leech is a freshwater amphibious worm, usually black or brown and about 10 cm in length. Of the more than 650 known species in the Hirudo genus, Hirudo medicinalis is used most often in medical therapy. The mouth of the leech has three jaws, each of which has about 100 tiny teeth. It also has a posterior suction device that it uses for stability.
Leech therapy involves an initial bite, which is usually painless (it's thought that leech saliva contains a mild anesthetic); an attachment period lasting 20 to 45 minutes, during which the leech sucks between 5 and 15 mL of blood; and a postattachment period, during which the site continues to bleed. The final stage provides the primary therapeutic benefit; it's caused by components in the leech's saliva, including hirudin, a protein anticoagulant that inhibits thrombin in the clotting process, as well as histamine-like substances that induce vasodilation.
The first medicinal leeches were obtained from streams and rivers, but today they're purchased from companies that function as leech farms. Facilities that don't have established protocols can follow the recommendations of these leech farms. It's advised that leeches be stored at a cool temperature (42° to 45°F [5° to 7°C] and no higher than 68°F [20°C]) in "leech mobile homes" containing distilled water treated with a special salt; some recommend changing the solution every other day.

PATIENT ASSESSMENT
Before leech therapy begins, a thorough assessment should be performed. Also, the nurse should be able to differentiate between arterial insufficiency and venous congestion. In arterial insufficiency, the tissue is pale, turgid, and cool to the touch; capillary refill is either slow (longer than two seconds) or absent. The tissue in venous congestion will have a purple appearance and be engorged, taut, and warm to the touch. Capillary refill will be brisk and instantaneous (less than one second). Although leech therapy may alleviate venous congestion, it's contraindicated in the presence of arterial insufficiency.
The nurse should also ask the patient whether she or he is taking vitamins, herbal supplements, or drugs; some may increase the risk of excessive bleeding or reduced immune response. For example, large doses of vitamin E can prolong prothrombin time, cause coagulopathy, and suppress immunity. Herbal supplements such as dong quai, garlic, ginger, gingko biloba, and ginseng and medications such as aspirin, heparin, warfarin, and nonsteroidal antiinflammatory drugs may increase the risk of hemorrhage.
Caffeine (found in coffee, tea, some sodas, and chocolate) must be eliminated in order to decrease the risk of vasoconstriction. Smoking (and even exposure to secondhand smoke) is absolutely contraindicated because the carbon monoxide and nicotine in tobacco are potent vasoconstrictors.
Contraindications. Leech therapy should not be used in patients who are immunocompromised, those with bleeding disorders such as hemophilia, and those with preexisting arterial insufficiency; all of these conditions pose an extreme risk of infection or excessive blood loss. A patient's refusal to accept blood transfusions is another contraindication, as is unstable medical status. And of course, patients who refuse leech therapy should not receive it.
Patient education. Few patients are willing to have contact with leeches without understanding the benefits of treatment. This obstacle is usually overcome with education.

COMPLICATIONS
Excessive bleeding can occur with leech therapy; it can be controlled by applying direct pressure or topical thrombin. Excessive blood loss may necessitate blood transfusion, so patients should be informed of the possibility. Allergic responses, including anaphylaxis, can also occur. Patients and their families should be alerted to watch for and report allergy symptoms. Scarring may also occur but is usually minimal.
Infection. The most serious complication of leech therapy is infection. The leech's digestive system contains Aeromonas hydrophila, a Gram-negative bacillus that enables the breakdown of ingested blood. Although most infections involving leech therapy are caused by A. hydrophila, infections with Serratia marcescens, A. sobria, and Vibrio fluvialis have been reported. Infections can arise from two to 11 days after therapy begins and can result in abscesses and cellulitis, which can progress in some cases to sepsis. In a five-year retrospective study, Sartor and colleagues found that infections arose in 4.1% of patients who received leech therapy. Prophylactic antibiotics are usually given: double coverage (two antibiotics) during therapy and single coverage (one antibiotic) for two weeks afterward. Established infection is treated with antibiotics such as third-generation cephalosporins, along with aminoglycosides, fluoroquinolones, tetracycline, or trimethoprim. Because infection is a serious adverse effect of leech therapy, the patient and family should also be instructed to observe for and report early signs and symptoms.

Acupuncture is an alternative medicine that treats patients by insertion and manipulation of needles in the body. Its proponents variously claim that it relieves pain, treats infertility, treats disease, prevents disease, promotes general health, or can be used for therapeutic purposes. Acupuncture typically incorporates traditional Chinese medicine as an integral part of its practice and theory. However, many practitioners consider 'Traditional Chinese Medicine' (TCM) to narrowly refer to modern mainland Chinese practice. Acupuncture in Japan and Korea, and to a certain extent Taiwan, diverged from mainland China in theory and practice. In European countries such as the UK almost half the practitioners follow these non-TCM practices. The most notable difference is that these other approaches often are primarily acupuncture, and do not incorporate Chinese herbal medicine. The term “acupuncture” is sometimes used to refer to insertion of needles at points other than traditional ones, or to applying an electric current to needles in acupuncture points. Acupuncture dates back to prehistoric times, with written records from the second century BCE. Different variations of acupuncture are practiced and taught throughout the world.
Ideas of what constitutes health and healing differ from concepts used in modern scientific, evidence based medicine. Traditional acupuncture was developed prior to the understanding of human anatomy and cell theory upon which modern biology is based, and there is no anatomical or scientific evidence for the existence of qi or meridians; concepts central to acupuncture theory.
The evidence for acupuncture's effectiveness for anything but the relief of some types of pain and nausea has not been established. Systematic reviews have concluded that acupuncture is no more effective than nonpenetrating stimulation of one point to reduce some types of nausea. Evidence for the treatment of other conditions is equivocal. Although evidence exists for a very small and short-lived effect on some types of pain, several review articles discussing the effectiveness of acupuncture have concluded it is possible to explain as a placebo effect and a 2011 review of review articles concluded except for neck pain acupuncture was of doubtful efficacy and accompanied by serious risks and adverse effects, including death. Publication bias is a significant concern when evaluating the literature. There is general agreement that acupuncture is safe when administered by well-trained practitioners using sterile needles.

Safety
Because acupuncture needles penetrate the skin, many forms of acupuncture are invasive procedures, and therefore not without risk. Injuries are rare among patients treated by trained practitioners in some countries. Sometimes, needles are required by law to be sterile, disposable and used only once; in some places, needles may be reused if they are first resterilized, e.g. in an autoclave. When needles are contaminated, risk of bacterial or other blood-borne infection increases, as with re-use of any type of needle.

Adverse events
Estimates of adverse effects due to acupuncture range from 671 to 1,137 per 10,000 treatments. A 2010 systematic review found that acupuncture has been associated with a possible total of up to 86 deaths over the years surveyed, most commonly due to pneumothorax. Some reported adverse effects include 50 cases of bacterial infections, and more than 80 cases of Hepatitis B since 1970. A 2011 review stated that "ninety-five cases of severe adverse effects including 5 fatalities" were evident in the literature reviewed. "Pneumothorax and infections were the most frequently reported adverse effects... serious adverse effects continue to be reported."
Other injury
Other risks of injury include: nerve injury, resulting from the accidental puncture of any nerve, brain damage or stroke, which is possible with very deep needling at the base of the skull, kidney damage from deep needling in the low back. Haemopericardium, or puncture of the protective membrane surrounding the heart, which may occur with needling over a sternal foramen.

Omitting modern medical care
Receiving alternative medicine as a replacement for standard modern medical care could result in inadequate diagnosis or treatment of conditions for which modern medicine has a better treatment record.
As with other alternative medicines, unethical or naive practitioners may also induce patients to exhaust financial resources by pursuing ineffective treatment. Profession ethical codes set by accrediting organizations such as the National Certification Commission for Acupuncture and Oriental Medicine require referrals to make "timely referrals to other health care professionals as may be appropriate." In Canada, public health departments in the provinces of Ontario and British Columbia regulate acupuncture.

What is herbal medicine?

Herbal medicine -- also called botanical medicine or phytomedicine -- refers to using a plant's seeds, berries, roots, leaves, bark, or flowers for medicinal purposes. Herbalism has a long tradition of use outside of conventional medicine. It is becoming more mainstream as improvements in analysis and quality control along with advances in clinical research show the value of herbal medicine in the treating and preventing disease.

What is the history of herbal medicine?

Plants had been used for medicinal purposes long before recorded history. Ancient Chinese and Egyptian papyrus writings describe medicinal uses for plants. Indigenous cultures (such as African and Native American) used herbs in their healing rituals, while others developed traditional medical systems (such as Ayurveda and Traditional Chinese Medicine) in which herbal therapies were used. Researchers found that people in different parts of the world tended to use the same or similar plants for the same purposes.

In the early 19th century, when chemical analysis first became available, scientists began to extract and modify the active ingredients from plants. Later, chemists began making their own version of plant compounds, and over time, the use of herbal medicines declined in favor of drugs.

Recently, the World Health Organization estimated that 80% of people worldwide rely on herbal medicines for some part of their primary health care. In Germany, about 600 - 700 plant-based medicines are available and are prescribed by some 70% of German physicians. In the last 20 years in the United States, public dissatisfaction with the cost of prescription medications, combined with an interest in returning to natural or organic remedies, has led to an increase in herbal medicine use.

How do herbs work?

In many cases, scientists aren' t sure what specific ingredient in a particular herb works to treat a condition or illness. Whole herbs contain many ingredients, and they may work together to produce a beneficial effect. Many factors determine how effective an herb will be. For example, the type of environment (climate, bugs, soil quality) in which a plant grew will affect it, as will how and when it was harvested and processed.

How are herbs used?

The use of herbal supplements has increased dramatically over the past 30 years. Herbal supplements are classified as dietary supplements by the U.S. Dietary Supplement Health and Education Act (DSHEA) of 1994. That means herbal supplements -- unlike prescription drugs -- can be sold without being tested to prove that they are safe and effective. However, herbal supplements must be made according to good manufacturing practices.

The most commonly used herbal supplements in the U.S. include echinacea (Echinacea purpurea and related species), St. John's wort (Hypericum perforatum), ginkgo (Ginkgo biloba), garlic (Allium sativum), saw palmetto (Serenoa repens), ginseng (Panax ginseng, or Asian ginseng; and Panax quinquefolius, or American ginseng), goldenseal (Hydrastis canadensis), valerian (Valeriana officinalis), chamomile (Matricaria recutita), feverfew (Tanacetum parthenium), ginger (Zingiber officinale), evening primrose (Oenothera biennis), and milk thistle (Silybum marianum).

Often, herbs may be used together because the combination is more effective and may have fewer side effects. Health care providers must take many factors into account when recommending herbs, including the species and variety of the plant, the plant's habitat, how it was stored and processed, and whether or not there are contaminants (including heavy metals and pesticides).

What is herbal medicine good for?

Herbal medicine is used to treat many conditions, such as asthma, eczema, premenstrual syndrome, rheumatoid arthritis, migraine, menopausal symptoms, chronic fatigue, and irritable bowel syndrome, among others. Herbal supplements are best taken under the guidance of a trained health care provider. Be sure to consult with your doctor or pharmacist before taking any herbs. Some common herbs and their uses are discussed below.

* Ginkgo (Ginkgo biloba) has been used in traditional medicine to treat circulatory disorders and enhance memory. Although not all studies agree, ginkgo may be especially effective in treating dementia (including Alzheimer's disease) and intermittent claudication (poor circulation in the legs). It also shows promise for enhancing memory in older adults. Laboratory studies have shown that ginkgo improves blood circulation by dilating blood vessels and reducing the stickiness of blood platelets. By the same token, this means ginkgo may also increase the effect of some blood-thinning medications, including aspirin. People taking blood-thinning medications should ask their doctor before using ginkgo.
* Kava kava (Piper methysticum) is said to elevate mood, well-being, and contentment, and produce a feeling of relaxation. Several studies have found that kava may be useful in the treatment of anxiety, insomnia, and related nervous disorders. However, there is serious concern that kava may cause liver damage. It's not clear whether the kava itself caused liver damage in a few people or whether it was taking kava in combination with other drugs or herbs. It's also not clear whether kava is dangerous at previously recommended doses, or only at higher doses. Some countries have taken kava off the market. It remains available in the United States, but the Food and Drug Administration (FDA) issued a consumer advisory in March of 2002 regarding the "rare" but potential risk of liver failure associated with kava-containing products.
* Saw palmetto (Serenoa repens) is used by more than 2 million men in the United States for the treatment of benign prostatic hyperplasia (BPH), a non-cancerous enlargement of the prostate gland. A number of studies suggest that the herb is effective for treating symptoms, including too-frequent urination, having trouble starting or maintaining urination, and needing to urinate during the night. But a well-conducted study published in the February 9, 2006 edition of the New England Journal of Medicine found that saw palmetto was no better than placebo in relieving the signs and symptoms of BPH.
* St. John's wort (Hypericum perforatum) is well known for its antidepressant effects. In general, most studies have shown that St. John's wort may be an effective treatment for mild to moderate depression, and has fewer side effects than most other prescription antidepressants. But the herb interacts with a wide variety of medications, including birth control pills, so it is important to take it only under the guidance of a health care provider.
* Valerian (Valeriana officinalis) is a popular alternative to commonly prescribed medications for sleep problems because it is considered to be both safe and gentle. Some studies bear this out, although not all have found valerian to be effective. Unlike many prescription sleeping pills, valerian may have fewer side effects such as morning drowsiness.
* Echinacea preparations (from Echinacea purpurea and other Echinacea species) may improve the body's natural immunity. Echinacea is one of the most commonly used herbal products, but studies are mixed as to whether it can help prevent or treat colds. A meta-analysis of 14 clinical studies examining the effect of echinacea on the incidence and duration of the common cold found that echinacea supplements decreased the odds of getting a cold by 58%. It also shortened the duration of a cold by 1.4 days.

Buying standardized herbal supplements helps ensure you will get the right dose and the effects similar to human clinical trials. Ask your doctor or pharmacist about which herbal supplements are the best choice for your health concerns.

Is there anything I should watch out for?

Used correctly, herbs can help treat a variety of conditions and in some cases may have fewer side effects than some conventional medications. But because they are unregulated, herbal products are often mislabeled and may contain additives and contaminants that aren' t listed on the label. Some herbs may cause allergic reactions or interact with conventional drugs, and some are toxic if used improperly or at high doses. Taking herbs on your own increases your risk, so it is important to consult your doctor or pharmacist before taking herbal medicines. Some examples of adverse reactions from certain popular herbs are described below.

* St. John's wort can cause your skin to be more sensitive to the sun's ultraviolet rays, and may cause an allergic reaction, stomach upset, fatigue, and restlessness. Clinical studies have found that St. John's wort also interferes with the effectiveness of many drugs, including the blood thinner warfarin (Couamdin), protease inhibitors for HIV, birth control pills, certain asthma drugs, and many other medications. In addition, St. John's wort should not be taken with prescribed antidepressant medication. The FDA has issued a public health advisory concerning many of these interactions.
* Kava kava has been linked to liver toxicity. Kava has been taken off the market in several countries because of liver toxicity.
* Valerian may cause sleepiness, and in some people it may even have the unexpected effect of overstimulating instead of sedating.
* Garlic, ginkgo, feverfew, and ginger, among other herbs, may increase the risk of bleeding.
* Evening primrose (Oenothera biennis) may increase the risk of seizures in people who have seizure disorders.

Some herbal supplements, especially those imported from Asian countries, may contain high levels of heavy metals, including lead, mercury, and cadmium.
